This session provides an overview of the 18+ Transition Program, focusing on services, alignment to our continuum of services, and best practices. Participants will explore how transition planning starts as early as elementary school, ensuring a seamless path to postsecondary success. Topics include individualized planning, vocational training, and collaboration with families and agencies. Attendees will leave with strategies to support students at all stages of their transition journey.
In this session, we’ll explore how to identify target behaviors and create clear, meaningful IEP goals. We’ll also look at the best ways to track behavior changes to get clear insights into goal progress. You’ll get useful tips, real-world examples, and a chance to collaborate and practice along the way.
